Sample Delivery

You may either ship or physically drop off your samples to the following address:

Please include the following information when shipping samples:

• Client Name
• Project Name/ID
• Sample ID for each sample
• Sample Matrix
• Elements to analyzed and method(s) to be used
• Chain of Custody Form

Also, please follow all proper procedures when shipping hazardous materials. If you have any concerns about shipment of material, one of our vendor partners, Keika Ventures offers an online training course on the subject.

Reports

To preserve natural resources, First Analytical Laboratories ships reports electronically but, if requested, we will provide send a hard copy of your report at no additional charge.

If you need to obtain a copy of a past report, please email or call with the following information:

• Your company project number and the approximate date the samples were submitted, or
• The FAL project number. The format is YMM##.

Electronic reports will be reissued at no cost.  There may be a small charge for the re-issue of hardcopy reports to cover shipping and handling.
